    Primary Pulmonary Synovial Sarcoma: A Case Report

    We report a rare case of primary synovial sarcoma of the lung. A 57-year-old man had a well-defined tumor in the right middle lobe seen on chest computed tomography, and underwent lobectomy. Grossly, the nonencapsulated tumor measured 4.5 cm in greatest diameter, with a solid and tan-white cut surface. Histologically, the tumor was mainly composed of a dense proliferation of spindle cells. Immunohistochemical studies were focally positive for epithelial membrane antigen, and diffusely positive for CD99 and Bcl-2. Cytokeratin, S-100 protein, desmin, smooth muscle actin, and CD34 were absent. SYT-SSX1 gene fusion transcript was detected by reverse transcription-polymerase chain reaction, which is diagnostic of primary synovial sarcoma of the lung. We also review the literature with regard to the clinicopathologic, immunohisto-chemical, and molecular studies of primary pulmonary synovial sarcoma

    Composite type A thymoma and diffuse large B-cell lymphoma

    AbstractThe concurrent occurrence of thymoma and diffuse large B-cell lymphoma in the thymus has not been previously reported. We describe a 74-year-old man who presented with general weakness, neck lymphadenopathy, night sweats, and body weight loss. A right anterior mediastinal mass was found on computed tomography of the chest. The immunohistochemical stains AE1/AE3, CD20, CD3, and MUM-1 confirmed the different components of the mediastinal tumor. A heavy-chain gene clonality assay and light-chain gene clonality assay confirmed the B-cell clonality of the mediastinal tumor and neck lymph node. The patient had received a complete course of chemotherapy, and the result of positron emission tomography–computed tomography showed complete remission. The pathologic report of this mass revealed composite type A thymoma and diffuse large B-cell lymphoma. If concurrent or composite thymoma and lymphoma are suspected, a thorough examination of the thymoma with a combination of ancillary studies is recommended to rule out the possibility of concurrent lymphoma

    A hybrid interpretable deep structure based on adaptive neuro‑fuzzy inference system, decision tree, and K‑means for intrusion detection

    For generating an interpretable deep architecture for identifying deep intrusion patterns, this study proposes an approach that combines ANFIS (Adaptive Network-based Fuzzy Inference System) and DT (Decision Tree) for interpreting the deep pattern of intrusion detection. Meanwhile, for improving the efciency of training and predicting, Pearson Correlation analysis, standard deviation, and a new adaptive K-means are used to select attributes and make fuzzy interval decisions. The proposed algorithm was trained, validated, and tested on the NSL-KDD (National security lab–knowledge discovery and data mining) dataset. Using 22 attributes that highly related to the target, the performance of the proposed method achieves a 99.86% detection rate and 0.14% false alarm rate on the KDDTrain+dataset, a 77.46% detection rate on the KDDTest+dataset, which is better than many classifers. Besides, the interpretable model can help us demonstrate the complex and overlapped pattern of intrusions and analyze the pattern of various intrusions

    On the combination of adaptive neuro-fuzzy inference system and deep residual network for improving detection rates on intrusion detection

    Deep Residual Networks (ResNets) are prone to overfitting in problems with uncertainty, such as intrusion detection problems. To alleviate this problem, we proposed a method that combines the Adaptive Neuro-fuzzy Inference System (ANFIS) and the ResNet algorithm. This method can make use of the advantages of both the ANFIS and ResNet, and alleviate the overfitting problem of ResNet. Compared with the original ResNet algorithm, the proposed method provides overlapped intervals of continuous attributes and fuzzy rules to ResNet, improving the fuzziness of ResNet. To evaluate the performance of the proposed method, the proposed method is realized and evaluated on the benchmark NSL-KDD dataset. Also, the performance of the proposed method is compared with the original ResNet algorithm and other deep learning-based and ANFIS-based methods. The experimental results demonstrate that the proposed method is better than that of the original ResNet and other existing methods on various metrics, reaching a 98.88% detection rate and 1.11% false alarm rate on the KDDTrain+ datase

    Areca Users in Combination with Tobacco and Alcohol Use Are Associated with Younger Age of Diagnosed Esophageal Cancer in Taiwanese Men

    BACKGROUND: Whether the habitual use of substances (tobacco, alcohol, or areca nut (seed of the Areca palm)) can affect the age of esophageal squamous cell carcinoma (ESCC) presentation has rarely been examined. METHODS: The study subjects were those who were males and the first time to be diagnosed as ESCC (ICD-9 150) and who visited any of three medical centers in Taiwan between 2000 and 2009. A standardized questionnaire was used to collect substance uses and other variables. RESULTS: Mean age (±SD) at presentation of ESCC was 59.2 (±11.3) years in a total of 668 cases. After adjusting for other covariates, alcohol drinkers were 3.58 years younger to have ESCC than non-drinkers (p = 0.002). A similar result was found among areca chewers, who were 6.34 years younger to have ESCC than non-chewers (p<0.0001), but not among cigarette smokers (p = 0.10). When compared to the group using 0-1 substances, subjects using both cigarettes and alcohol were nearly 3 years younger to contract ESCC. Furthermore, those who use areca plus another substance were 7-8 years younger. Subjects using all three substances had the greatest age difference, 9.20 years younger (p<0.0001), compared to the comparison group. CONCLUSION: Our findings suggest that habitually consuming tobacco, alcohol, and areca nut can influence the age-onset of ESCC. Since the development of ESCC is insidious and life-threatening, our observation is worthy to be reconfirmed in the large-scale and long-term follow-up prospective cohort studies to recommend the screening strategy of this disease

    Habitat Complexity Affects the Structure but Not the Diversity of Sessile Communities on Tropical Coastal Infrastructure

    Increasing human population, urbanisation, and climate change have resulted in the proliferation of hard coastal infrastructure such as seawalls and breakwaters. There is increasing impetus to create multifunctional coastal defence structures with the primary function of protecting people and property in addition to providing habitat for marine organisms through eco-engineering - a nature-based solutions approach. In this study, the independent and synergistic effects of physical complexity and seeding with native oysters in promoting diversity and abundances of sessile organisms were assessed at two locations on Penang Island, Malaysia. Concrete tiles with varying physical and biological complexity (flat, 2.5 cm ridges and crevices, and 5 cm ridges and crevices that were seeded or unseeded with oysters) were deployed and monitored over 12 months. The survival of the seeded oysters was not correlated with physical complexity. The addition of physical and biological complexity interacted to promote distinct community assemblages, but did not consistently increase the richness, diversity, or abundances of sessile organisms through time. These results indicate that complexity, whether physical or biological, is only one of many influences on biodiversity on coastal infrastructure. Eco-engineering interventions that have been reported to be effective in other regions may not work as effectively in others due to the highly dynamic conditions in coastal environment. Thus, it is important that other factors such as the local species pools, environmental setting (e.g., wave action), biological factors (e.g., predators), and anthropogenic stressors (e.g., pollution) should also be considered when designing habitat enhancements. Such factors acting individually or synergistically could potentially affect the outcomes of any planned eco-engineering interventions.</jats:p

    Factors Influencing COVID-19 Vaccination Intention among University Students : A Cross-Sectional Study in Malaysia

    Vaccination is crucial in controlling the spread of the coronavirus disease 2019 (COVID-19) that triggered the pandemic, but herd immunity can only work with high vaccination coverage in the population. This study aims to measure the COVID-19 knowledge level and determine the factors influencing COVID-19 vaccination intention among university students in Malaysia. A cross-sectional online survey was carried out with 1,274 Malaysian university students in July 2021. Univariate and multivariate analyses were employed to examine the relationships between the study variables. Results showed that the majority of university students had an acceptable level of knowledge of COVID-19. The knowledge, risk perception of COVID-19, social norms, and perceived benefit of COVID-19 vaccination were positively associated with vaccination intention. However, perceived trust in information sources of COVID-19 vaccination and the government's response to COVID-19 did not affect the university students’ desire to receive the vaccination. These findings are essential for health policymakers and healthcare providers to implement evidence-based interventions to increase COVID-19 vaccination uptake among university students

    Enhancing Uptake of Nature-Based Solutions for Informing Coastal Sustainable Development Policy and Planning: A Malaysia Case Study

    Nature-based Solutions (NbS) have been advocated to protect, sustainably manage, and restore natural or modified ecosystems, simultaneously providing human well-being and biodiversity benefits. The uptake of NbS differs regionally with some countries exhibiting greater uptake than others. The success of NbS also differs regionally with varying environmental conditions and social-ecological processes. In many regions, the body of knowledge, particularly around the efficacy of such efforts, remains fragmented. Having an “inventory” or “tool box” of regionally-trialed methods, outcomes and lessons learnt can improve the evidence base, inform adaptive management, and ultimately support the uptake of NbS. Using Malaysia as a case study, we provide a comprehensive overview of trialed and tested NbS efforts that used nature to address societal challenges in marine and coastal environments (here referring to mangroves, seagrass, coral reefs), and detailed these efforts according to their objectives, as well as their anticipated and actual outcomes. The NbS efforts were categorized according to the IUCN NbS approach typology and mapped to provide a spatial overview of IUCN NbS effort types. A total of 229 NbS efforts were collated, representing various levels of implementation success. From the assessment of these efforts, several key actions were identified as a way forward to enhance the uptake of Nature-based Solutions for informing coastal sustainable development policy and planning. These include increasing education, training, and knowledge sharing; rationalizing cooperation across jurisdictions, laws, and regulations; enhancing environmental monitoring; leveraging on existing policies; enabling collaboration and communication; and implementing sustainable finance instruments. These findings can be used to inform the improved application and uptake of NbS, globally.</jats:p
